Blue Cross of Idaho began in 1945 as the Idaho Hospital Service (IHS). They specialized in pre-paid hospital care for businesses and employees, including many Idaho mining operations.

In their first year, the IHS enrolled more than 10,000 members. Their second year saw 35,000 new members.

In 1948, IHS opened their pre-paid hospital plans for individuals. Soon after, their product portfolio expanded to provide outpatient services such as x-rays and lab tests, accident insurance, and major medical coverage.

The Idaho Hospital Service became Blue Cross of Idaho in 1973. Their headquarters is in the city of Meridian.
